Summary
In a 74-year-old man with well-known cutaneous and gastrointestinal symptoms of neurofibromatosis
(von Recklinghausen's disease), three neurofibromatous tumors developed, causing a
stenosis of the esophageal lumen 20 cm below the tooth line. By endoscopy polypectomy,
it was possible to remove all three tumors by electroresection, to extract them, and
thus to restore a free passageway through the esophagus. Indications for treatment,
technical methods, and possible complications are discussed. The reported case is,
to our knowledge, unique in the literature, both in the severe involvement of the
esophagus in neurofibromatosis and in the successful therapeutic polypectomy of the
esophagus by electroresection.
Zusammenfassung
Bei einem 74jährigen Mann mit bekannter kutaner und gastrointestinaler Manifestation
einer Neurofibromatose (Morbus von Recklinghausen) kam es durch 3 Neurofibromknoten
in der Speiseröhre zu einer Stenose des Ösophagus 20 cm unterhalb der Zahnreihe. Durch
eine zweizeitige endoskopische Polypektomie gelang es, alle 3 Tumoren mittels Elektroresektion
zu entfernen und so eine freie Speiseröhrenpassage wiederherzustellen. Technisches
Vorgehen, Indikation und Komplikationsmöglichkeiten werden diskutiert. Der geschilderte
Fall scheint der erste in der Weltliteratur zu sein, bei dem eine derart schwere Beteiligung
des Ösophagus bei einer Neurofibromatose auftrat, und der erste, bei dem eine erfolgreiche
therapeutische Polypektomie mittels Elektroresektion aus dem Ösophagus durchgeführt
wurde.
